Job Title: Community Health Nurse - Band 6

We are seeking a highly skilled and experienced Community Health Nurse to join our team at Berkshire Healthcare NHS Foundation Trust. As a Community Health Nurse, you will be responsible for providing high-quality care to patients in their own homes, promoting independence and self-care, and working collaboratively with other healthcare professionals to achieve the best possible outcomes.

Key Responsibilities:
  • Provide comprehensive assessments and care plans for patients with complex needs
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Work collaboratively with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
  • Conduct regular reviews and updates of patient care plans to ensure ongoing relevance and effectiveness
Requirements:
  • Registered Nurse - Adult qualification and active on the Mentor Register
  • Evidence of leadership and management development, including coaching qualification or willingness to undertake
  • At least 2 years of Community Nursing experience at Band 6
