Ackerman Pierce Ltd is seeking a dedicated and experienced Adults First Response Social Worker to join our team. As a key member of our social care team, you will be responsible for providing high-quality care management services to adults in need.
Key Responsibilities:
- Short-term Interventions: Develop and implement effective short-term interventions to address the needs of adults in crisis.
- Care Planning: Create and manage care plans that meet the assessed needs of adults, ensuring the delivery of services within available financial resources.
- Case Management: Manage a caseload of adults, providing regular assessments, reviews, and updates to ensure the best possible outcomes.
- Collaboration: Work closely with multi-disciplinary teams, including healthcare professionals, to ensure comprehensive care and support.
- Record Keeping: Maintain accurate and up-to-date records of adult cases, ensuring compliance with regulatory requirements.
Requirements:
- Qualifications: Post-qualifying experience as an Adults First Response Social Worker, with a strong understanding of adult social care principles and practices.
- Registration: HCPC registration as a social worker.
- Experience: A minimum of 1 year post-qualifying experience in the UK social care industry.
- DBS: Enhanced DBS/CRB clearance (Ackerman Pierce Ltd can assist with the application process).
- Eligibility: Eligibility to work in the UK.
