**Job Summary:**

We are seeking an experienced and compassionate Care Manager to oversee our domiciliary care services. As a key member of our team, you will be responsible for coordinating care plans, managing a team of care providers, and ensuring the well-being of our clients receiving home-based care.

**Key Responsibilities:

  • Develop individualized care plans for clients receiving domiciliary care
  • Supervise and support a team of care providers, ensuring compliance with care plans and regulations
  • Conduct regular assessments of clients' needs and adjust care plans accordingly
  • Collaborate with healthcare professionals, families, and external agencies to provide comprehensive care
  • Maintain accurate records and documentation of care services

**Requirements:**

  • Degree in Health & Social Care or a related field
  • Previous experience in a care management role, preferably in domiciliary care
  • Excellent communication and interpersonal skills
  • Ability to lead and motivate a team in a fast-paced environment
  • Knowledge of relevant healthcare regulations and best practices in domiciliary care

**Estimated Salary:** £35,000 - £45,000 per annum.
