We are seeking a skilled and empathetic Care Professional to join our team at OP Care Services Limited. As a Care Professional, you will be responsible for delivering individualized care to our service users, promoting their independence and well-being.
About the Role:- You will work closely with the care team to assess service users' needs, develop personalized care plans, and implement effective care strategies.
- Maintain accurate records of service users' care plans, progress, and any changes to their needs.
- Care Planning: Develop and review care plans in collaboration with service users, their families, and the care team.
- Service User Support: Provide emotional and practical support to service users, helping them maintain their independence and confidence.
- Medication Management: Administer medication to service users, following established protocols and procedures.
- Personal Care: Assist service users with daily living activities, including personal hygiene, toileting, and mobility support.
- Assessment and Review: Conduct regular assessments to identify service users' changing needs, updating their care plans accordingly.
