About the Team

As a Healthcare Support Professional, you will be part of a dynamic and compassionate team dedicated to delivering high-quality patient care. This role requires empathy and understanding, as well as the ability to work autonomously and make decisions based on patient interactions.

Main Responsibilities:

  • Supporting patients in their own homes, promoting independence and improving quality of life.
  • Working collaboratively with multidisciplinary teams to develop and implement person-centred care plans.
  • Conducting regular assessments and reviews to ensure patient needs are met and outcomes are achieved.

Requirements:

  • Numeracy and Literacy Level 2 (GCSE 6-9)
  • Care certificate
  • Level 3 vocational qualification or equivalent knowledge and experience in clinical care duties.
  • Excellent analytical and judgement skills, with the ability to make decisions based on patient interactions and observations.
  • Strong timekeeping, planning, and prioritisation skills.
