We are seeking an experienced Care Coordinator to join our team in South Birmingham, working closely with healthcare professionals to deliver high-quality patient care. As a key member of our Multi-Disciplinary Team, you will contribute to tackling health inequalities, particularly among individuals with long-term conditions.
Key Responsibilities- Administrative Support:
- Act as administrative support to the practice safeguarding lead, ensuring relevant correspondence is passed to the safeguarding lead as necessary.
- Collaborate with care homes and supported living establishments to arrange annual reviews and coordinate access for GPs.
- Desirable Qualifications:
- Completed BCU Care Coordination Training Course.
- Vulnerable adults awareness and experience caring for the elderly.
- Knowledge of social services structures and continuing care criteria.
- Your Contribution:
- You will work collaboratively with healthcare professionals to identify frequent flyers and new service users, utilizing risk stratification tools to inform decision-making.
- You will assist in directing patients with limited English proficiency, ensuring information is accessible to them or signposting them to appropriate services.
