Care Coordinator

Salary: £25,000 - £32,000 per annum
Rivendell, a part-time Intensive Support Practitioner role at Quarriers, is seeking a qualified individual to provide support to children and young people with complex needs.
This role requires a driver with access to their own transport due to the service's idyllic location. The successful candidate will work closely with the team to provide tailored support, ensuring each person's needs are met in a safe and enjoyable environment.
Job Description:
- To work with children and young people with complex needs, providing support in their daily lives.
- To contribute to a collaborative team environment, sharing knowledge and expertise to ensure high-quality care.
- To maintain accurate records of support provided, using effective communication skills to record and report as required.
Required Skills and Qualifications:
- Square 1 SVQ Level 3 in Health & Social Care (Children & Young People) and HNC.
- Experience working with children and families in similar settings.
- Excellent communication skills for effective recording and reporting.
- Possession of a driver's license with access to a vehicle.
