We are seeking a skilled and compassionate Adult Nurse Manager to join our team at Leaders in Care, a progressive provider of complex care packages within the community. This exciting opportunity is based in the Wembley area, where we have secured new contracts during an period of growth.

About the Role

This challenging and rewarding position involves providing face-to-face support to adult clients and their families, while creating personalized care plans tailored to each individual's needs. You will work closely with a dedicated team of carers, training and supporting them with their clinical skills and activities.

Key Responsibilities
  • Develop and deliver comprehensive care plans that meet the unique needs of clients
  • Effectively lead and manage multidisciplinary teams through strong communication and collaboration
  • Provide ongoing assessment, monitoring, and support of clients' physical and emotional needs
  • Deliver high-quality nursing interventions, including medication management
  • Foster strong relationships with clients' families and engage them in the care planning process
  • Collaborate with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke care packages
Requirements and Qualifications

To be successful in this role, you will possess:

  • An active RGN registration with the NMC
  • A supportive management style that brings out the best in others
  • Up-to-date experience in tracheostomy care and ventilator management within the last 2 years
  • Advanced communication skills for effective liaison with patients, families, and multidisciplinary teams
  • Band 5 or Band 6 nursing experience, with evidence of leadership and management skills
  • A full UK driving license (desirable but not essential)
