About the Role
">We are seeking a highly skilled and dedicated Registered Nurse to join our community nursing team. This is an excellent opportunity for a motivated individual to work in a dynamic environment, delivering high-quality patient care and making a real difference in the lives of our patients.
The successful candidate will be responsible for assessing, planning, implementing, and evaluating patient care plans, working closely with multidisciplinary teams to ensure comprehensive care.
As a community staff nurse, you will have the opportunity to work independently, using your clinical skills and expertise to provide holistic care to patients in their own homes or in local community settings.
This role requires strong communication and interpersonal skills, as well as the ability to work flexibly and respond to changing circumstances.
Key Responsibilities
- To assess the health needs of patients and develop individualized care plans
- To deliver high-quality patient care, adhering to relevant standards and guidelines
- To work collaboratively with other healthcare professionals to ensure seamless care delivery
- To maintain accurate and up-to-date records, in accordance with NMC standards
- To participate in quality improvement initiatives and contribute to the development of best practices within the team
Requirements
- NMC registration as a qualified adult nurse (RGN)
- Post-registration experience in a community nursing setting
- Ability to perform complex assessments and develop effective care plans
- Experience of working with palliative care patients
- Strong communication and interpersonal skills
Salary: £34,000 - £43,041 per annum
