About the Role:
We are seeking an experienced and caring individual to join our team as a Care and Support Specialist. This is an excellent opportunity to work with individuals and their families, providing person-centered care and support that enables them to live independently.
Your Key Responsibilities:
- Develop and implement personalized care plans for clients, taking into account their unique needs and goals.
- Provide emotional and practical support to clients, including assistance with daily living activities, mobility, and medication management.
- Work collaboratively with other healthcare professionals to ensure comprehensive care and support.
- Engage with clients in social activities and promote community integration.
Requirements:
- 2 years or more experience in a similar role.
- SVQ 2 or equivalent in Health or Social Care.
