About the Role

We are seeking a skilled and compassionate Health and Wellbeing Practitioner to join our team at The Forward Trust. As a key member of our integrated healthcare framework, you will provide vital support to prisoners aged 18 and over, promoting holistic recovery and health and wellbeing.

Key Responsibilities
  • Manage a caseload and provide appropriate harm reduction advice and guidance
  • Deliver structured 1:1 and group-work sessions for substance misuse and low-level mental health service users
  • Carry out assessments, risk assessments, and comprehensive assessments to inform treatment journeys
  • Facilitate substance misuse and mental health groups, and wellbeing and recovery programmes
  • Support service users in developing release plans and provide transitional support from prison to the community
Requirements
  • Experience working in a Health or Justice setting
  • Knowledge of substance misuse and low-level mental health issues
  • Demonstratable experience of carrying out risk assessments and comprehensive assessments
  • Significant experience of carrying out psychosocial interventions, including motivational interviewing techniques
  • A thorough knowledge of and commitment to the 12-step process of recovery from addiction
