Job Title: Health and Wellbeing Practitioner

Location: HMP Downview

Salary: £25,154 - £27,947 per annum

We are seeking a skilled Health and Wellbeing Practitioner to join our team at HMP Downview. As a Health and Wellbeing Practitioner, you will work within an integrated healthcare framework to provide support to prisoners aged 18 and over.

Key Responsibilities:

  • Manage a caseload of prisoners with substance misuse and low-level mental health needs
  • Provide harm reduction advice and guidance, assessments, and care plans
  • Deliver structured 1:1 and group-work sessions for substance misuse and low-level mental health service users
  • Support service users in developing release plans and providing transitional support from prison to the community

Requirements:

  • Experience working in a Health or Justice setting
  • Knowledge of substance misuse and low-level mental health issues
  • Ability to carry out risk assessments, comprehensive assessments, and design and implement SMART care plans
  • Significant experience of carrying out psychosocial interventions, including motivational interviewing techniques
