**Job Description**

We are seeking an experienced Skilled Nursing Professional - Community Setting to join our NHS community health team. As a key member of our team, you will provide high-quality nursing care to patients in their own homes, promoting independence and self-care.

**Key Responsibilities**

- Provide skilled nursing care based on principles of best practice and clinical evidence
- Work collaboratively with multidisciplinary teams to assess and address patients' physical, psychological, and social care needs
- Develop and implement individualised care plans that promote monitoring and promotion of self-care to prevent unnecessary hospital admissions
- Act as a patient advocate, facilitating patient choice regarding nursing care and promoting independence and self-care, as appropriate
- Participate in the implementation and monitoring of Trust clinical and operational guidelines and policies
