About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our team at the East Lancashire Alliance. As a key member of our primary care team, you will provide high-quality, patient-centered care to patients in care homes and the community.

Key Responsibilities
  • Provide expert clinical care to patients in care homes, managing acute, non-acute, and chronic disease conditions.
  • Work collaboratively with the multi-disciplinary team to deliver safe and high-quality care.
  • Carry out clinical nursing practice at an advanced level, using expert clinical skills and critical judgments to deliver appropriate and safe care.
  • Triage and treat patients, performing specialist assessments and making referrals as necessary.
  • Coordinate the planning and delivery of holistic care, ensuring patients and carers are involved.
  • Prescribe and review medication, using evidence-based practice and national protocols.
  • Promote health and well-being through health promotion, health education, screening, and therapeutic communication skills.
Professional Role
  • Maintain up-to-date skills and knowledge, staying aware of professional issues at an advanced level.
  • Assess the effectiveness of own clinical practice and care delivery through self-reflection and peer review.
  • Promote evidence-based practice through the use of latest research-based guidelines and practice-based research.
  • Participate in continuing professional development opportunities to maintain up-to-date evidence-based knowledge and competence.
Teaching and Mentoring Role
  • Promote a learning environment for patients, care home staff, and other health professionals.
  • Contribute to the planning and implementation of teaching for existing staff, such as medical students, nursing students, and healthcare assistants.
Health and Safety/Risk Management

The post holder must comply with the practice's health and safety policies, following agreed safe working procedures and reporting incidents using the organization's incident reporting system.

Confidentiality

The post holder must maintain confidentiality regarding all aspects of patient care, adhering to the Data Protection Act and the Access to Health Records Act.
