We are seeking a dedicated Community Support Specialist to join our team at Polaris Community, working on a casual basis in London. This role offers flexibility with no contractual obligation, allowing you to work the hours you can commit to.

As an Outreach Service Provider, you will travel around London, providing support to young people in their supported living homes and the community. Your primary responsibility will be to deliver a responsive, customized, and quality service, focusing on the safety and well-being of the individuals you support.

Your key duties include:

  • Maintaining positive relationships with young people, staff, and professionals through effective communication
  • Ensuring continuous support is provided, adhering to person-centred risk assessments and support plans
  • Liaising with external agencies, such as police and Emergency Duty Team (EDT)

To succeed in this role, you should have experience supporting vulnerable young people, be flexible with your availability, including weekend work, and hold a Residential Childcare/Families Practitioner Diploma or equivalent qualification. A full driving license and access to a vehicle are also essential.
