Role Overview:

This is a fantastic opportunity to join our team as a Care Package Coordinator, supporting a client with complex needs in Leigh and the surrounding area. As a key member of our team, you will play a vital role in ensuring the client receives exceptional care and support.

Responsibilities:

  • Develop and implement personalized care plans that meet the client's specific needs.
  • Collaborate with healthcare professionals to ensure seamless care coordination.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
  • Maintain accurate records of client interactions and progress.

Requirements:

  • 2+ years of experience in care coordination or equivalent.
  • Relevant qualifications in health and social care (NVQ Level 3 desirable).
  • Full UK driving license.
  • Enhanced DBS clearance and professional references.
