Social Care Professional

We are looking for an experienced Social Care Professional to join our Adults Team. As a key member of the team, you will be responsible for providing high-quality care and support to individuals with learning disabilities.
Key Responsibilities- Conduct assessments and reviews to identify individual needs
- Develop and implement personalized care plans
- Work collaboratively with other agencies to provide comprehensive support
- Post-qualifying experience as a Qualified Social Worker - Adults Learning Disabilities Team within the UK Social Care industry
- HCPC registration
- Enhanced DBS/CRB (or willingness to obtain)
£38,000 - £48,000 per annum, depending on experience, plus benefits package including opportunities for training and career progression
