Clinical Pharmacist

Holland House Surgery is seeking a highly skilled Clinical Pharmacist to join our team and contribute to the delivery of our medicines optimisation goals.

1. Medication Reviews: Conduct face-to-face and remote medication reviews with patients to ensure safe and effective medication use.
2. Medication Queries: Respond to medication-related queries from patients and other clinicians, providing expert advice and guidance.
3. Medicines Optimisation: Work with patients and healthcare professionals to optimise medication regimens and manage chronic diseases.
4. Acute Medication Requests: Manage acute medication requests and ensure timely and safe medication delivery.
5. Medicines Reconciliation: Conduct medicines reconciliation to identify and resolve medication discrepancies.
6. Audit Work: Participate in audit work to monitor and improve medication use and safety.
7. Team Collaboration: Liaise with our established PCN pharmacy team to ensure seamless communication and collaboration.

While an existing prescribing qualification is desirable, support to achieve this would be provided.
