Job SummaryWe are seeking an experienced Advanced Community Nurse Practitioner to join our team as a Clinical Lead for our Evening and Nights Community Nursing service. As a Clinical Lead, you will be responsible for providing direction and leadership to the team, ensuring the delivery of high-quality, person-centered care to our patients.
Key Responsibilities- Provide Direction and Leadership: To provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives.
- Focus on Person-Centered Care: To focus on the delivery of person-centered care, ensuring that our patients receive the best possible care and support.
- Effective Caseload Management: To provide effective, evidence-based care to a defined population through caseload management and monitoring of care standards.
- Leadership and Supervision: To provide leadership, including clinical supervision, to team members and support the Out of Hours Community Team managerial lead.
To be successful in this role, you will need:
- Advanced Community Nurse Practitioner Qualification: You will need to have a relevant qualification and experience as an Advanced Community Nurse Practitioner.
- Leadership Experience: You will need to have experience of leading a team and providing direction and leadership.
- Excellent Communication Skills: You will need to have excellent communication skills, both written and verbal.
- Ability to Work Independently: You will need to be able to work independently and as part of a team.
