About the Job

We are looking for a highly skilled Social Services Coordinator to join our Adult Social Care Team in Wokingham Borough Council. As a key member of our team, you will be responsible for conducting assessments and reviews, developing support plans, and working collaboratively with individuals, their carers, and other stakeholders.

As a Social Services Coordinator, you will work closely with vulnerable adults, identifying their needs and delivering tailored support to promote independence, well-being, and safety. You will have a strong understanding of relevant legal frameworks, policies, and guidance, ensuring that your practice is informed and effective.

Key Responsibilities:

  • To conduct comprehensive assessments and reviews of vulnerable adults' needs, identifying areas for support and intervention.
  • To develop and implement personalized support plans, promoting independence, well-being, and safety.
  • To work collaboratively with individuals, their carers, families, and other professionals to gather information, share knowledge, and coordinate services.
  • To maintain accurate records, reporting on progress, and evaluating the effectiveness of support plans.

Requirements:

  • Degree or equivalent in a relevant field (e.g., social work, psychology, or related discipline).
  • Minimum 1 year post-qualification experience in adult social care, with a focus on supporting vulnerable adults.
  • Demonstrable expertise in risk management, communication, and interpersonal skills.
  • Full driving license and access to own vehicle (for travel between locations).

About the Salary and Benefits:

The salary range for this role is £41,511 - £47,754 per annum, plus benefits including annual leave entitlement, pension scheme enrollment, employee assistance program, local & lifestyle discounts, and use of a free onsite gym.
