About the Role

As a Holistic Care Specialist at Sonderwell, you will play a vital role in delivering exceptional care to our clients with complex needs. This rewarding opportunity allows you to make a real difference in people's lives, enabling them to live independently and with dignity.

Key Responsibilities
  • Provide personalized care to meet the unique needs of each client, including assistance with daily living activities, personal care, and mobility support.
  • Collaborate with multidisciplinary teams to develop and implement comprehensive care plans that promote client well-being and quality of life.
  • Conduct regular assessments to identify changing client needs and adapt care plans accordingly.
Requirements
  • Prior experience in providing care to individuals with complex needs, preferably in a residential or community setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to meet the changing needs of clients.
  • Basic Life Support (BLS) certification and willingness to undertake additional training as required.
