Job Overview

Key Responsibilities:
• Provide clinical expertise and leadership to ensure safe, high-quality care delivery
• Manage a complex caseload of clients requiring clinical support in the community
• Develop and implement individualized care plans tailored to each client's needs
• Collaborate with multidisciplinary teams, including healthcare professionals and family members

Requirements:
• Registered Nurse with an active PIN
• Minimum two years' clinical nursing experience, including community or hospital settings
• Management of clinical risk management and individualized care planning
• Experience in safeguarding adults and children
• Effective communication skills - verbal and written
• Ability to work independently and as part of a team, self-motivated
• Valid UK driving license and access to a car (highly desirable)
