We are seeking an experienced and skilled Advanced Care Practitioner to join our team at West View Millennium Surgery. As a key member of our primary care team, you will be responsible for delivering high-quality patient care, working autonomously and in collaboration with other healthcare professionals to assess, diagnose, and treat patients.
Key Responsibilities- Manage a clinical caseload, dealing with presenting patients' needs in a primary care setting.
- Lead the implementation of clinical contractual needs of the practice, including Quality and Outcome Framework and Enhanced Services.
- Provide expert professional advice to patients, carers, and colleagues, ensuring the maintenance of clinical excellence.
- Develop new and innovative concepts, models, methods, and practices to deliver new and improved care services to meet the needs of service users.
- Provide education and training to other staff and students.
- Undertake audit and education as part of the role.
- Experience of working in a primary care environment.
- Post Graduate Diploma/Degree (Advanced Practice Qualification).
- Minimum four years' experience in primary care.
- Demonstrate evidence of study at master's level/master's degree.
- Clinical skills training/qualification and experience.
