About the Role

We are seeking a skilled and experienced Adult Senior Practitioner to join our team at the Leeds and York Partnership NHS Foundation Trust.

Job Description

The successful candidate will work as an integral member of our clinical team, providing high-quality care to individuals with acute mental health problems. The role involves undertaking crisis assessments, implementing home-based treatment interventions, and working closely with inpatient and community services.

You will be responsible for promoting effective team working, ensuring seamless communication, and maintaining accurate records. As a senior clinician, you will also provide leadership, management, and supervision to junior staff members.

Key Responsibilities:

  • Undertake crisis assessments and implement home-based treatment interventions
  • Work closely with inpatient and community services to promote early discharge and reduce admissions
  • Provide leadership, management, and supervision to junior staff members
  • Maintain accurate and contemporaneous records

Requirements:

  • Qualification in Mental Health Nursing, Social Work, or Occupational Therapy
  • Post-registration experience working with adults with acute mental health problems
  • Ability to consult, liaise, and negotiate with service users, carers, and other professionals
  • Valid driver's license and access to a car
