Community Health Nurse

We are seeking a highly motivated and experienced Registered Nurse to join our Reactive Emergency Assessment Community Team (REACT) as a key member of our multidisciplinary team.
Job SummaryThe successful candidate will be responsible for delivering high-quality patient-focused care to patients in the East Suffolk community, working in an internal rotation and traveling throughout the area to provide acute care in patients' homes.
Main Responsibilities- Assess, plan, and deliver healthcare for patients with long-term conditions, palliative care, and rehabilitation needs to achieve quality of life and independence.
- Work with the multidisciplinary team to deliver Outpatient Parenteral Antibiotic Therapy (OPAT) in the community to facilitate discharge from hospital or prevent unnecessary admission.
- Contribute to the prevention of unnecessary hospital admissions by working within the integrated team.
- Collaborate with healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
