Community Health Nurse

We are seeking a highly skilled and motivated Registered Nurse to join our Reactive Emergency Assessment Community Team (REACT) as a Community Health Nurse. The successful candidate will be responsible for delivering high-quality patient care in the community, working closely with a multidisciplinary team to provide seamless care to our service users.
Key Responsibilities- Assess, plan, and deliver healthcare for patients with long-term conditions, palliative care, and rehabilitation needs, to achieve quality of life and independence where possible.
- Work with the multidisciplinary team to deliver Outpatient Parenteral Antibiotic Therapy (OPAT) in the community, facilitating discharge from hospital or preventing unnecessary admission to hospital.
- Work within the integrated team to prevent unnecessary admission to hospital.
- Collaborate with all healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.

Requirements- Effective observational and reporting skills.
- Ability to be focused and undertake patient care, input accurate data/patient records.
- A team player.
- Able to work flexibly to accommodate patient/service needs.
- IT and standard keyboard skills.
- Able to manoeuvre patients using handling aids.
- Car owner/driver/access to a motor vehicle (role will involve travel to other sites that cannot be easily accessed by public transport means).
- Leadership Skills.
- RN Qualification (Part 1 of register) or equivalent.
- Current NMC registration.
