As a Band 5 Community Staff Nurse, you will play a vital role in providing high-quality nursing care to patients in their own homes, residential homes, and community settings.
Key Responsibilities- Assess, prescribe, and deliver care packages for patients
- Work collaboratively with the integrated team to facilitate early discharge from hospital and prevent unnecessary admission
- Provide clinical assessment for incoming nursing referrals
- Liaise and communicate with healthcare professionals, statutory, and non-statutory agencies to provide a seamless service to patients
- Registered Adult Nurse Qualification
- Diploma or 1st Level Degree
- Current NMC Registration
