About the Role

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our team at Avon and Wiltshire Mental Health Partnership NHS Trust. As a key member of our community mental health team, you will be responsible for providing high-quality care and support to individuals with mental health needs in the community.

Key Responsibilities
  • Conduct comprehensive mental health assessments and develop individualized care plans
  • Provide evidence-based interventions and treatments to individuals with mental health needs
  • Work collaboratively with other healthcare professionals to ensure seamless care and support
  • Develop and maintain strong relationships with service users, their families, and carers
  • Participate in multidisciplinary team meetings and contribute to the development of care plans
Requirements
  • Registered Mental Health Nurse or equivalent qualification
  • Experience working in community mental health settings
  • Strong communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Knowledge of mental health legislation and policies
