About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team in the Older Adult Liaison service. As a key member of our team, you will be responsible for providing mental health assessments and care to older adults in the acute general hospital setting.

Key Responsibilities
  • Conduct comprehensive mental health assessments and develop treatment plans for older adults with complex mental health needs.
  • Provide liaison and consultation services to general hospital colleagues to ensure seamless care coordination.
  • Collaborate with multidisciplinary teams to provide education and clinical supervision to registered and unregistered staff.
  • Represent mental health services within the general hospital and facilitate communication between services.
  • Develop and maintain relationships with primary care and secondary mental health services to ensure high-quality patient care.
Requirements
  • Registered Nurse (RMN) with current NMC registration or equivalent qualification (OT or Social Worker).
  • Knowledge of safety assessments and management in relation to suicide and suicide prevention.
  • Knowledge of the Mental Health Act (1983), Mental Capacity Act, and Deprivation of Liberty Safeguards.
  • Knowledge of delirium and dementia care pathways.
  • Teaching and assessing qualification (or equivalent) desirable.
  • Experience working in Older Adult Mental Health settings and Liaison teams in acute general hospitals desirable.
