About the Role

The Older Peoples Rehabilitation and Assessment Service based at Leith Community Treatment Centre is seeking an experienced Deputy Charge Nurse with a passion for delivering high quality care for older people.

You will lead a nurse team as part of the Multi-disciplinary team who provide outpatient assessment and interventions for older people living in North Edinburgh.

The service works with older people, GPs and wider health and social care professionals to improve health and functional ability and reduce future risk of hospital admission.

Key Responsibilities
  1. Lead a nurse team to provide high quality care for older people
  2. Work collaboratively with the Multi-disciplinary team to provide outpatient assessment and interventions
  3. Develop and implement care plans to improve health and functional ability
  4. Collaborate with GPs and wider health and social care professionals to reduce future risk of hospital admission
