We are seeking an experienced Advanced Nurse Practitioner to join our team at Rural Rother PCN. As a key member of our primary care network, you will play a vital role in delivering high-quality, patient-centered care to our community.
Key Responsibilities- Evaluate patients with complex care needs, using physical assessment skills and independent nurse prescribing (V300) to create personalized care plans.
- Assess and manage acute and non-complex medical needs of patients who are housebound, palliative care, and vulnerable, within your scope of practice and knowledge.
- Work with other local service providers, such as GP surgeries, community services, and secondary care, to ensure good communication and coordination of care.
- Implement and evaluate individualized treatment plans for chronic disease patients, identifying and managing patients at risk of developing long-term conditions.
- Provide advanced, specialist nursing care to patients as required, in accordance with evidence-based guidelines and best practices.
- Advanced Practice Qualification (Masters Level) or equivalent experience, with a level 7 module that gives access to the e-portfolio route of registration as an Advanced Nurse Practitioner.
- Qualified Independent Nurse Prescriber V300, with experience of prescribing and undertaking medication reviews.
- Experience of working in primary care, urgent care, or A&E, with a strong understanding of holistic assessments and treatment plans.
