Advanced Nurse Practitioner for Primary Care Network

About the Role
Rural Rother PCN is seeking an experienced Advanced Nurse Practitioner to join our team. As an Advanced Nurse Practitioner, you will work autonomously to evaluate patients with complex care needs, using physical assessment skills and independent nurse prescribing (V300) to create or update personalised care plans.
Key Responsibilities
- Evaluate patients with complex care needs using physical assessment skills and independent nurse prescribing (V300)
- Create or update personalised care plans for vulnerable patients living in the local primary care network area
- Triage and manage acute and non-complex medical needs of patients who are housebound, palliative care, and vulnerable
- Work with other local service providers to ensure good communication between the multi-disciplinary team
- Provide advanced, specialist nursing care to patients as required, in accordance with evidence-based evidence, NICE, and the NSF

Rural Rother PCN is based in an office in Battle, East Sussex. We have an excellent team of clinical pharmacists, physios, paramedic practitioners, social prescribers, and care navigators who all provide a service to our patients in the rural Rother area.
