As a key member of our Community Based Pulmonary Rehabilitation Programme, you will be responsible for providing high-quality, responsive, specialist respiratory community physiotherapy services to patients in their own homes or other appropriate locations.
Main Duties- Provide physiotherapy services to patients with Chronic Obstructive Pulmonary Disease (COPD) and other respiratory conditions.
- Develop and implement individualized treatment plans to improve patients' physical function, mobility, and overall quality of life.
- Work collaboratively with other healthcare professionals, including doctors, nurses, and social workers, to ensure comprehensive care for patients.
- Conduct assessments, evaluations, and interventions to promote patient independence and self-management.
- Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improving services.
- Registered Physiotherapist with the Health and Care Professions Council (HCPC).
- Postgraduate experience of working as part of a physiotherapy team and multidisciplinary team in Respiratory Medicine and/or Pulmonary Rehabilitation.
- Full UK driving licence and access to a car for use to travel across sites and perform domiciliary visits.
