About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Cheshire West and Chester Council. As a Support Worker, you will play a vital role in providing high-quality, person-centred care to adults with learning disabilities, autism, dementia, and the elderly.

Key Responsibilities

  • Caring for and supporting service users by following care plans and risk assessments to meet their physical, emotional, and mental health needs.
  • Keeping full, accurate, and up-to-date records, including risk assessments, care, and support plans for service users and daily records of events while on duty.
  • Supporting service users to manage their own emotions and behaviours, encouraging independence, and promoting social, emotional, and physical well-being.
  • Administering or supporting with medications, as required.
  • Assisting service users with advice and support to manage their finances and maintain their homes.
  • Acting as a Key Worker, using a person-centred approach, to complete monthly reviews of risk assessments, care, and support plans, and contributing to six-monthly reviews.
  • In line with care plans, assisting with risk assessments for events and access to the community for service users, individually or in groups, leisure facilities, libraries, and other activities.
  • Building links within the community to enable service users to access facilities and services, social, leisure, employment, and education.
  • Safeguarding service users at all times, taking necessary actions immediately, and reporting any concerns or issues to management.
  • Working positively with managers, colleagues, and other professionals, such as social workers and health professionals, to fully support service users and encourage feedback to improve care and support.
