Support Care Assistant

We are seeking a highly motivated and compassionate Support Care Assistant to join our team. As a Support Care Assistant, you will be responsible for providing high-quality, person-centred care to adults with learning disabilities, autism, dementia, and the elderly in their own homes or in our day and respite services.
Key Responsibilities- Deliver person-centred care and support to service users, ensuring their physical, emotional, and mental health needs are met.
- Collaborate with multidisciplinary teams to develop and implement individual care plans, risk assessments, and support plans.
- Provide emotional and practical support to service users, promoting independence, choice, and control.
- Administer or support with medications, as required.
- Support service users with daily living tasks, such as personal care, meal preparation, and household management.
- Assist service users with accessing community facilities and services, promoting social inclusion and community engagement.
- Maintain accurate and up-to-date records, including risk assessments, care plans, and daily records of events.
- Be committed to delivering person-centred care and support to adults with learning disabilities, autism, dementia, and the elderly.
- Have excellent communication and interpersonal skills, with the ability to work effectively with service users, families, and multidisciplinary teams.
- Hold a Level 2 QCF/NVQ in Social Care (or be willing to work towards it).
- Be able to work flexibly, including evenings, weekends, and bank holidays, as required.
