About the Job

As a Care Coordinator, your primary responsibilities will include:

  • Scheduling routine clinic activities and referring patients to relevant services.
  • Providing exceptional customer service skills, responding to queries, and communicating effectively with patients and care professionals.
  • Preparing and updating referral documentation and client databases.
  • Carrying out follow-up calls to referrers to ensure high-quality referrals and effective communication.
