At Newbury Street Practice, we are seeking a highly skilled and experienced Advanced Nurse Practitioner to take on the role of Clinical Lead. This is an exciting opportunity for a motivated and ambitious individual to lead our team of Advanced Nurse Practitioners and contribute to the delivery of high-quality patient care.

Key Responsibilities

  1. Manage patients presenting with a range of acute and chronic medical conditions, providing expert advice and guidance.
  2. Develop and implement individualised treatment plans for patients with chronic diseases, working closely with other healthcare professionals to ensure seamless care.
  3. Identify and support patients at risk of developing long-term conditions, implementing strategies to prevent adverse effects on their health.
  4. Provide advanced nursing care to patients, utilising evidence-based practice and adhering to clinical guidelines.
  5. Undertake the collection of pathological specimens and process and interpret test results as required.
  6. Maintain accurate and contemporaneous clinical records, adhering to legislation and practice policies.
  7. Ensure the effective use of SNOMED CT codes and contribute to the development of practice targets and quality improvement initiatives.
  8. Support the team in dealing with clinical emergencies and provide supervision and guidance as needed.
  9. Recognise and refer patients presenting with mental health needs, working closely with external services and agencies as required.
  10. Contribute to the development and implementation of practice policies and procedures, ensuring compliance with local and national guidelines.
  11. Delegate clinical responsibilities appropriately, ensuring safe practice and adherence to scope of practice.
  12. Support the clinical team with safeguarding matters, adhering to local and national policies and procedures.
  13. Deliver opportunistic health promotion and education to patients, promoting healthy lifestyles and disease prevention.

  2. Be verified against the ANP roadmap and recognised as a subject matter expert in advanced nursing practice.

  4. Assess the effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
  5. Participate in research and utilise the audit cycle to evaluate the quality of work and implement improvements as needed.

Administrative Requirements

  1. Produce accurate, contemporaneous and complete records of patient consultations, adhering to legislation, policies and procedures.
  2. Be aware of data protection and confidentiality issues, particularly within a GP practice.
  3. Use technology and appropriate software to aid management in planning, implementation and monitoring of care, presenting and communicating information.
  4. Review and process data using accurate read codes, ensuring easy and accurate information retrieval for monitoring and audit processes.
