Clinical Medication Review Specialist

We are seeking an experienced General Practice pharmacist to join our team as a Clinical Medication Review Specialist. This is a challenging and rewarding role that requires a strong clinical background, excellent communication skills, and the ability to work effectively in a multidisciplinary team.
Key Responsibilities:- Lead and coordinate clinical medication reviews for patients with chronic conditions, including asthma, diabetes, and hypertension.
- Work closely with practice staff to develop and implement medication management plans, ensuring safe and effective prescribing practices.
- Provide education and training to practice staff on various aspects of medication management, including side effects, interactions, and dosing instructions.
- Collaborate with other healthcare professionals to improve patient outcomes and reduce hospital readmissions.
- Minimum 5 years of experience as a practicing pharmacist in General Practice.
- Certificate in Non-medical Prescribing.
- Experience of using prescribing qualification in General Practice.
- Evidence of further education in clinical pharmacy, such as completion of NHS Pathway Training for Practice Pharmacists or a Clinical Pharmacy Diploma.
The role is based in London, with easy access to public transportation.
