Lead Clinical Learning Disability Nurse Practitioner

Job Description:
We are seeking an experienced and dedicated nurse practitioner to join our multidisciplinary team in providing high-quality services to individuals with learning disabilities and complex health needs.
The successful candidate will be responsible for assessing eligibility criteria under the NHS National Framework for Continuing Healthcare, designing care packages, and procuring services for individuals with a learning disability who have complex health needs. This includes the CHC process and Section 117 aftercare.
As a key member of our team, you will be expected to carry a caseload of people with complex physical and mental health needs, care manage, review, and monitor their packages of care at least annually unless earlier re-assessment is required.
You will work closely with local authorities on behalf of Devon ICB and contribute to the effective provision of services across the multidisciplinary team, ensuring that other teams in wider LD services are supported when needed.
All staff working within the Learning Disability service are responsible for providing and maintaining high-quality supportive relationships that acknowledge the individual rights and personal dignity of the service user, encouraging individual opportunities for growth physically, mentally, and emotionally.
Main Duties and Responsibilities- Assess eligibility criteria under the NHS National Framework for Continuing Healthcare.
- Design and implement care packages for individuals with a learning disability who have complex health needs.
- Procure services for individuals requiring CHC and Section 117 aftercare.
- Care manage, review, and monitor packages of care for individuals with complex health needs.
