Advanced Pharmacist

We are seeking an experienced and skilled Advanced Pharmacist to join our team at Oxford University Hospitals NHS Foundation Trust. As a key member of our clinical pharmacy service, you will play a crucial role in providing high-quality patient care and contributing to the development of our pharmaceutical care services.

Main Responsibilities
  1. Provide a clinical pharmacy service to the gastroenterology ward, working closely with healthcare professionals to optimize patient care and outcomes.
  2. Support the development and implementation of holistic, specialist pharmaceutical care services for patients receiving high-risk, high-cost medication.
  3. Contribute to the provision of clinical pharmacy services to outpatients and endoscopy, ensuring seamless care transitions and optimal patient outcomes.
  4. Deliver a ward-based clinical pharmacy service, adhering to Trust standards and guidelines, and providing expert advice on medication management, side effects, and monitoring requirements.
