Job Summary:

We are seeking a skilled and compassionate Support Worker to join our team at Torfaen County Borough Council. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals in their own homes, enabling them to live independently and safely.

About the Role:

The ideal candidate will have a basic understanding of care in the community and experience of caring for people in a paid or voluntary capacity. You will be comfortable working on your own, have a flexible approach, and be able to maintain good working relationships with service users.

Key Responsibilities:

  • Provide person-centred care and support to individuals in their own homes, promoting their independence and well-being.
  • Work collaboratively with the service user, their carers, and other professionals to develop and implement individual care plans.
  • Maintain accurate records of service user interactions, including care plans, risk assessments, and incident reports.
  • Participate in regular team meetings and contribute to the development of the service.

Requirements:

  • QCF Level 2 or NVQ 2 in Social Care, or a willingness to complete.
  • Full Driving Licence and own transport.
  • Basic understanding of Domiciliary Care Services.
  • Be registered or willing to register as a Support Worker with Social Care Wales.
