About the Role:

We are seeking a dedicated and compassionate Support Worker to join our team in Torfaen. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals in their own homes, enabling them to live independently and safely.

Key Responsibilities:

  • Provide personal care and support to service users, including assistance with daily living tasks and medication management.
  • Develop and maintain positive relationships with service users, their families, and other professionals.
  • Work collaboratively with the multidisciplinary team to ensure comprehensive care and support plans are in place.
  • Contribute to the development and implementation of individualized care plans, taking into account the unique needs and preferences of each service user.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in social care.

Requirements:

  • Basic understanding of care in the community and experience of caring for people in a paid or voluntary capacity.
  • Comfortable working on your own and in a team environment, with excellent communication and interpersonal skills.
  • Current driving license and use of transport essential.
  • Qualified to NVQ level II in Care or willing to work towards Level 2 Diploma in Health & Social Care (QCF).
  • Willing to register with Social Care Wales.
