Job Overview

We are seeking a skilled Mental Health Practitioner to join our team in Primary Care, working as part of a General Practice team in South Bristol. This is an exciting opportunity to provide expert care and support to our local population.

As a Mental Health Practitioner, you will play a key role in our community mental health services, working collaboratively with Primary Care, voluntary services, and Avon & Wiltshire Mental Health Partnership NHS Trust to provide a personalized, preventative, and proactive integrated approach.

This is a part-time opportunity with flexible working considered. We value our staff and offer a range of development opportunities, including leadership programs, coaching and mentoring, and access to training via the Workforce Development Fund.

Key Responsibilities

As a Mental Health Practitioner, your key responsibilities will include:

  • Providing expert mental health care and support to patients in Primary Care
  • Working collaboratively with the General Practice team to develop and implement care plans
  • Participating in multidisciplinary teams to provide a comprehensive and integrated approach to mental health care
  • Developing and maintaining relationships with patients, families, and carers to ensure effective care and support
