About the Role
We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Andover Health Centre Medical Practice. As a key member of our Primary Care Network, you will play a vital role in supporting patients with complex social needs and promoting holistic approaches to health and wellbeing.
Key Responsibilities
- Develop and maintain strong relationships with patients, families, and carers, providing person-centred support and empowering individuals to take control of their health and wellbeing.
- Work collaboratively with local agencies, community groups, and stakeholders to promote social prescribing and reduce health inequalities.
- Provide advice and signposting services, connecting individuals with community resources and statutory services to address their social and emotional needs.
- Support the development of community groups and VCSE organisations, promoting micro-commissioning and small grants to enhance community provision.
- Collect and track data to assess the impact of social prescribing on patient outcomes, ensuring effective monitoring and evaluation of services.
Requirements
- Ability to listen, empathise, and provide person-centred support in a non-judgemental way.
- Strong awareness and understanding of the wider determinants of health and their impact on communities.
- Experience of working in a community development context, adult health, and social care, or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to maintain confidentiality and adhere to data protection legislation.
Desirable Qualifications and Experience
- Relevant qualifications set out by the Personalised Care Institute or willingness to study for these.
- Experience of supporting people with their mental health, either in a paid, unpaid, or informal capacity.
- Knowledge of community development approaches, motivational coaching, and interview skills.
Working Arrangements
This is a full-time position, working 37.5 hours per week. You will be required to work flexibly, including occasional evenings and weekends, to meet the needs of patients and services.
