About the Role:

We are seeking an experienced Social Worker to join our Disabled Children's Team in Hampshire, covering Test Valley Directorate. As a Social Worker, you will be passionate about providing quality services to children with disabilities and their families, delivered within a strength-based approach model.

You will be responsible for working with children and their families to undertake holistic social work assessments to identify risk and need, and work with multi-agency networks to develop and review plans that address and mitigate concerns in a timely manner.

You will be part of a stable, friendly team working alongside our Occupational Therapy service, and will be required to liaise with health and education professionals to develop support packages for children with complex needs.

Key Responsibilities:

  • Undertake holistic social work assessments to identify risk and need
  • Work with multi-agency networks to develop and review plans
  • Liaise with health and education professionals to develop support packages
  • Work with children and their families to increase independence and promote family resilience

Requirements:

  • Social Work qualification
  • Prior experience of working with children with disabilities
  • Ability to understand and navigate multi-agency systems and processes
