We are seeking an experienced and skilled Advanced Clinical Research Practitioner to join our team at West Kent Primary Care.
Key Responsibilities:- Work in our Same Day Access Hub, seeing patients for urgent on-the-day issues, and collaborate with the PCN team of GP supervisors and Paramedic Practitioners.
- Conduct cross-cover with the Frailty Visiting Team, visiting housebound patients with urgent on-the-day issues.
- Deliver safe and high-quality care as part of a multi-disciplinary team.
- Coordinate the planning and delivery of care, ensuring patient and carer involvement where required.
- Assess, diagnose, and treat patients in practice premises or their own homes, referring to appropriate agencies when necessary.
- Maintain effective communication with the primary care team and external stakeholders.
The postholder has a duty to safeguard and promote the welfare of vulnerable adults and children. They must be familiar with and follow local policies on safeguarding vulnerable adults and child protection procedures.
Leadership and Management:The successful candidate will take responsibility for their own learning and performance, participating in clinical supervision and acting as a positive role model. They will contribute to the development of local guidelines, protocols, and standards, and promote the role of the Advanced Clinical Practitioner in general practice.
Research:The postholder will critically engage in research activity, using evidence-based strategies to improve and enhance patient care. They will participate in clinical governance activity, contributing to the improvement of health outcomes through audit, risk management, and the Quality and Outcomes Framework.
Quality:The successful candidate will strive to maintain quality within the practice, participating in clinical governance activity and contributing to the improvement of health outcomes. They will alert other team members to concerns about risk, quality, and safety, and contribute to the effectiveness of the team by reflecting on their own and team activities.
