Care Manager

Active Care Group's Care in the Home division is seeking an experienced Care Manager to join our team on a part-time basis. As a Care Manager, you will have ownership of care packages across Swindon and surrounding areas, working closely with the Business Development Manager to review suitable referrals and produce assessment reports for care packages. You will also be responsible for building excellent working relationships with clients, families, and external bodies, ensuring high-quality care at all times.
Key Responsibilities:
- Have ownership of care packages in your area

Requirements:
- NVQ 4 or above in Health and Social Care
- Full clean driving licence with access to own car
- Passion for delivering high-quality client-focused care
- Experience in care at Team Leader level or above
- Previous management experience would be an advantage
