**Job Description:** We are seeking a highly skilled and dedicated Senior Practitioner to join our team in Oxfordshire. As a key member of our Assessment Team, you will be responsible for assessing the needs of vulnerable adults and providing effective support.

**Responsibilities and Duties:

  • Conduct thorough assessments of individual needs and develop tailored care plans.
  • Collaborate with colleagues and external agencies to deliver comprehensive support services.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as necessary.
  • Supervise students and support newly qualified social workers.
  • Prepare reports and attend meetings to ensure the best interests of service users.

**Estimated Annual Salary:** £47,500, based on experience.
