About the Role

Affinity Trust is seeking an experienced Individual Support Coordinator to join our team. As an Individual Support Coordinator, you will work closely with individuals with Autism who live in their own flats in Bristol, providing tailored support to promote independence and social connections.

Job Description

The successful candidate will have a strong background in supporting individuals with Learning disabilities and Autism, with a focus on Positive Behaviour Support. You will work collaboratively with the individual, their family, and other professionals to develop and implement personalized support plans that meet their unique needs and goals.

Key Responsibilities:
  • Providing one-to-one support to individuals with Autism, focusing on building confidence and life skills.
  • Coordinating and participating in community-based activities, promoting social inclusion and integration.
  • Maintaining accurate records and reporting any changes or concerns to relevant stakeholders.
Requirements

To be successful in this role, you will need:

  • A full UK driving licence.
  • Previous experience working with individuals with Learning disabilities and Autism.
  • An understanding of Positive Behaviour Support principles.
  • A creative and flexible approach, with the ability to work effectively in a dynamic environment.
