Job Title: Support Coordinator

We are seeking a highly skilled and dedicated Support Coordinator to join our team at Creative Support. As a Support Coordinator, you will play a vital role in providing outcome-focused care and support to individuals who are at risk of homelessness and in need of housing-related provision.

Key Responsibilities:

  • Manage a team of recovery workers to provide direct support to service users in accordance with their holistic support plans.
  • Provide one-off interventions as well as longer-term support to help service users build resilience, local connections, and access local service provision.
  • Develop clear exit strategies to enable service users to maintain tenancies in the local community.
  • Work collaboratively with the team to promote inclusion, independence, and wellbeing among service users.

Requirements:

  • Essential experience in a similar role, with a strong understanding of housing-related provision and support.
  • Access to a car for work-related travel.
  • Ability to work flexibly, including evenings and weekends, to meet the needs of the service.
